Standard Forms

Below is a list of forms accompanied by a short description. Click on the document to access the form.

Addendum -  to be used to add additional terms to an existing contract or when there is not sufficient room on a purchase contract to include all the terms being offered.

Buyer Advisory - this advisory discuss some of the more common issues that a buyer may decide to investigate or verify concerning a home purchase.

Buyer Inspection Notice and Seller's Response - is a form that directly addresses the inspection period in the Residential Purchase Contract. It provides for the buyer to respond to a home inspection whether it's to request repairs, waive the inspection, or to cancel the contract. The form further provides for the seller to respond to any requests from the buyer.

Compensation Agreement (FSBO)
- Agreement for brokers/agents representing buyers who want to purchase FSBOs (For Sale By Owner) and when there isn't a listing; provides for compensation for the seller to the buyer's broker/agent.

Counter Offer -  to be used to make counter offers to initial purchase offers.

H.O.A./Condominium/Planned Community Addendum - is a disclosure to the buyer about what information the Buyer should receive, by Arizona law, and the time frames in which the buyer has to reply.

HUD-1 Statement - use by the Title Company to summarize the charges and proceeds from the sale.

Lead-Based Paint Disclosure -  is to be used to meet requirements for lead-based paint disclosure for residential sales.

Loan Status Report - is to be used by either the buyer or the buyer's lender to disclose to the seller the status of the buyer's loan application.

Open House Sign-in Sheet - may all of your guests register. You are letting strangers into your home; protect yourself by always having another adult with you while doing an open house.

Polybutylene Piping - use for homes built between January 1, 1978 and July 31, 1995 that have polybutylene pipes.

Pool Disclosure - use when a house being sold has a private pool.

Residential Resale Purchase Contract -  used for the sale of residential real property and includes provisions for financing information.

Residential Seller Property Disclosure Statement (SPDS)
- for sellers to complete when offering their home for sale. It provides for the sellers to disclose information about the condition of the property being offered for sale.
